What is an Affidavit of Due Diligence Template?

An Affidavit of Due Diligence attests to the forensic genealogist's efforts to identify the person(s) of interest in response to a particular legal question. The affidavit template provides a format for reporting the results of a genealogical investigation to show the court that a legal obligation to determine kinship or identity has been met. The template includes the boilerplate text with tabbed fields to save time when creating each new affidavit. It also incorporates paragraph numbers and exhibit numbers that automatically renumber as new paragraphs or exhibit numbers are inserted. The same template may be used as an affidavit of kinship, affidavit of family tree, affidavit of identity, or affidavit of heirship, etc., as appropriate to the specific case and locality. I currently have Affidavit of Due Diligence Templates available in Microsoft Word for Macintosh and Windows platforms. The Affidavit of Due Diligence Template is a user form with auto-numbered sequences for the exhibit and image lists.

Note: At this present time, 26 February 2023, Microsoft Word for Mac 2016, 2019, 2021 and 365 do NOT support custom toolbars. They are working on it but until then there are two options for Word Mac 2016:

  • Use keyboard shortcuts to trigger the macros until Word toolbars are completed-
  • Create custom tabs with the macros. See my new support page for info-
